Misc spec coverage improvements (#2821)

* Dont use raise_error by itself (avoids warning)

* Add coverage for AccountFilter

* Improve coverage and refactor for Subscription#lease_seconds

* Improve coverage and refactor for NotificationMailer

* Simplify assignment of min/max threshold on subscription
